Referrals Made Easy

Thank you for referring your patient to the CSANT CT Imaging Center. The physicians here value the relationships we have with our referring physicians and welcome any feedback or comments you may have regarding our scanning services. We take care to reinforce your role as your patient's primary care doctor and our role as a consulting physician.
 
Clear and appropriate communication with our referring physicians is important to us. After the patient's visit, we will forward a summary of diagnostic reports immediately. For your convenience you can either call us or complete our Referral Form and fax it to our office. Please do not submit patient information via e-mail as it is not secure.

Our Referring Physicians Service is available Monday through Friday from 8:30 a.m. to 4:30 p.m. at

972-566-7733. You can request to speak to a physician directly or call the direct line below to schedule

an appointment.

CT Office:

• Call 214-473-2810 (to speak with the CT technologist)

Hours: 8:30 am to 3:00 pm.
